JOB PURPOSE/SUMMARY
WFS provides Representative Payee services to consumers who have a mental health diagnosis or intellectual developmental disorder (IDD) and require assistance to manage their social security benefits and to meet their basic financial needs. The Financial Service Coordinator provides direct financial case management services to these consumers.
MINIMUM QUALIFICATIONS
Education and Experience:
  • Associate's degree in a human services field and 5 years of experience in direct care OR
  • Bachelor's degree in a social work or social sciences related field and a minimum of 1 year of experience in direct care
Licenses, Certifications & Clearances:
  • Act 33/34 Clearances
  • FBI Clearance
  • Valid State of PA Class 1 Drivers License

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Assesses individual strengths and needs through Representative Payee referral process, review of social and clinical information, and face-to-face individual budget meetings.
  • Conducts initial budget meeting with the consumer and referral source, reviews and assesses overall financial picture, prioritizes needs, opens case in electronic record according to regulations, and presents needs and budget strategy to Supervisor and related team members.
  • Develops, prepares, and implements an individualized budget for each person receiving Representative Payee services. Ensures face-to-face and/or phone contact with the consumer, as needed, to assist with financial concerns, build on strengths, and achieve budgeting goals.
  • Completes daily/routine tasks to ensure the consumer obtains the funds designated in his/her budget plan.
  • By being aware of all mandated social security regulations, complies with reporting the necessary information to the appropriate party within identified time frames.
  • By being aware of all services received by the consumer (e.g., Service Coordination, Supportive Living, Outpatient, etc.), participates in case reviews as part of an integrated treatment team. Meets with the consumer, their family, other agency team members, and/or outside parties in and out of the office.
  • Through being knowledgeable of benefits, is able to access benefits for which consumers may be eligible, including, but are not limited to, Medicare, Medicaid, food stamps, Supplemental Security Income (SSI), Social Security, veteran's benefits, Women, Infants, and Children (WIC) program, rent and utility rebates, etc.
  • Ensures that the consumer's financial needs are met through the utilization of natural supports (e.g., family, friends), community resources, or specialized services (e.g., debt consolidation agencies, attorneys). Assists consumers and other team members in identifying, linking, accessing, and coordinating such resources.
  • Investigates financial resources and communicates this information to colleagues as a member of an integrated treatment team. Maintains current awareness of available financial resources, including possible funders, eligibility requirements for such programs, and alternative resources.
  • Participates in interdisciplinary team meetings to ensure appropriateness and responsiveness of services in relation to the consumer's needs. Communicates with agency program staff regarding consumer's needs and/or issues.
  • Advocates for the consumer to ensure responsiveness from natural, community, and specialized services, including providing information, removing barriers, creating options, and resolving financial problems.
  • Maintains accurate and timely written documentation of activity on the consumer's behalf. Adheres to regulations for documentation. Updates consumer's electronic record as needed. Reviews charts for compliance with regulations. Prepares for formal audits.
  • Utilizes the designated amount of available time in productive service to consumers. Records time spent on individual and collateral contacts.
  • Coordinates and monitors specialized financial arrangements with personal care homes, Dom care facilities, assisted living facilities, etc. Updates arrangements, as necessary, and informs relevant team members of changes.
  • Completes forms, outcomes information, satisfaction surveys, and other program related material within the designated time frames.
  • Addresses all issues of fraud waste and/or abuse if they arise and proactively looks for ways to prevent instances of fraud, waste, and abuse in the service delivery system and work systems of the agency as described in the agency Compliance Plan.
  • Works in conjunction with program personnel to review any changes in Representative Payee benefit payments to assure that changes are appropriate and based on proper information.
  • Recommends and establishes individual burial or special needs trusts to ensure consumers stay under established resource limits.
  • Completes routine paperwork.
