What You'll Do:

As a Care Manager serving the North Shore or Gloucester/Cape Ann area, you will manage a field-based caseload of older adults living at home, in assisted living, or in skilled nursing settings. Assess the

Whole Picture:

Evaluate medical conditions, functional safety, and Social Determinants of Health (SDOOH), including housing safety, nutrition, and social isolation.

Build & Execute Care Plans:

Create client-centered plans of care, facilitate family team meetings, and adapt strategies as client needs evolve. Bridge the

Gaps:

Coordinate closely with physicians, specialists, hospitals, VNAs, assisted living communities, memory care resources, and elder services agencies.

Advocate During Transitions:

Ensure seamless handoffs when clients move between settings (like transitioning from a rehab facility back home) so nothing falls through the cracks. Close the

Loop:

Follow up on referrals, orders, appointments, and medical equipment (DME). You'll be the person who asks, " Did this actually get done? " and make sure it does.

Keep Families Connected:

Maintain detailed, timely clinical notes in our care platform so long-distance family members stay fully informed and reassured.

Compensation:

Competitive pay-per-visit structure ranging from $35.00 to $45.00 per hour (averaging $40.00 per hour across visits and coordination work).

Reimbursement:

Mileage reimbursement provided for local travel.

Schedule:

Core operating hours are Monday through Friday, 8:30am to 5:00pm, with high flexibility to build a daily schedule around your clients and personal availability. Who Are You? You are a compassionate, resourceful self-starter who isn't intimidated by complex systems and knows how to advocate firmly while remaining warm, professional and respectful.

Professional Credentials:

RN, LPN, or Licensed Social Worker (LCSW, LSW, BSW) strongly preferred. Degree in Nursing, Social Work, Healthcare Administration, or a related field required.

Senior Care Experience:

3+ years of experience working with older adults in home care, care management, assisted living, skilled nursing, or geriatric health settings.

System Knowledge:

Familiar with senior care options, VNA service limits, assisted living/SNF regulations, and discharge planning workflows.

Tech Component:

Comfortable using Google Workspace (Docs, Sheets, Drive, Calendar), Microsoft Office, and mobile electronic care platforms.

Logistics:

Valid driver's license, reliable personal vehicle for daily local travel across the North Shore or Cape Ann, and ability to pass CORI background check.
