Job Description
Overview We are seeking a dynamic and dedicated CCM/RPM Liaison to serve as a vital connector between clinical care management teams and remote patient monitoring (RPM) programs. In this role, you will facilitate seamless communication, coordinate demos, and ensure optimal utilization of RPM technology to enhance patient outcomes. This position offers an exciting opportunity to work in remote healthcare facilities throughout the Bay Area. Duties Act as the primary point of contact between care management teams and RPM service providers, ensuring clear communication and coordination. Facilitate patient onboarding, including demos, appointment scheduling, and road plan strategy. Collaborate with healthcare teams to develop demos that incorporate services involving CCM and remote patient monitoring. Build business relations. Assist with discharge planning to CCM at hospitals and post-acute care facilities. Collect health record documentation, document patient interactions, and ensure compliance with HIPAA regulations and medical terminology standards. Gain CCM/RPM consent from patients at various facilities and provide documentation to CRM portal. Requirements Have reliable transportation. Background in healthcare facilities such as post-acute care settings, hospitals, medical offices or outpatient clinics. Familiarity with managed care processes, discharge planning procedures, medical records management, and utilization management protocols. Experience working with diverse patient populations including, geriatrics, or patients requiring acute or outpatient care. Excellent patient service skills with the ability to perform comprehensive patient assessments and coordinate complex case management activities. Ability to navigate hospital systems efficiently while maintaining HIPAA compliance; proficiency in medical records documentation is essential.
