Our organization unifies and leads providers to improve the lives of people experiencing homelessness by sharing expertise, maximizing resources, and advocating for change.

Position Summary:

The Homeless Alliance of Western New York (HAWNY) is seeking a credentialed clinical professional to join a community-based initiative serving individuals experiencing homelessness with complex behavioral health, medical, and housing needs.

This position will work directly with individuals experiencing homelessness to provide clinical assessment, diagnosis, intervention, and connection to appropriate treatment and housing resources. The clinician will work in the community alongside HAWNY staff, outreach teams, healthcare and behavioral health providers, shelters, hospitals, and other community partners.

Primary Responsibilities:

Identify clinical conditions and barriers impacting an individual's ability to obtain and maintain housing. Provide crisis assessment, intervention, and de-escalation. Develop individualized clinical recommendations and service plans. Connect individuals to appropriate mental health, substance use, medical, psychiatric, and supportive services. Complete or support clinical documentation necessary for referrals to appropriate housing and behavioral health programs. Provide field-based services in encampments, hospitals, community settings, and other locations where individuals experiencing homelessness are engaged. Collaborate with outreach teams, healthcare providers, behavioral health agencies, housing providers, and other community partners. Participate in case conferencing and coordinated planning for individuals with complex needs. Maintain timely and accurate clinical and program documentation. Support individuals throughout the process of connecting to treatment, services, and permanent housing.

Required Candidate Qualities:

Candidates must be appropriately licensed and credentialed in New York State and legally authorized to perform clinical assessment and diagnosis within their professional scope of practice. HAWNY will consider qualified candidates with credentials including: Licensed Clinical Social Worker (LCSW) Licensed Mental Health Counselor (LMHC) Psychiatric-Mental Health Nurse Practitioner (PMHNP) Other appropriately licensed and credentialed clinical professionals with relevant diagnostic authority may be considered.

Candidates must also have:

Experience conducting clinical assessments and working with behavioral health needs. Knowledge of mental health and substance use disorders. Ability to work independently in community-based settings. Strong crisis intervention and engagement skills. Ability to work effectively with individuals experiencing complex needs and significant barriers to care. Valid driver's license and reliable transportation.

Working Conditions:

This is a direct service role that includes community-based outreach, office-based documentation, and occasional in-person meetings or events. The position may involve exposure to varying weather conditions during field work. Some flexible scheduling may be required to accommodate client needs.
