Job Description

A nonprofit services organization in New York City is seeking a new Intensive Case Manager for a hybrid role in the Bronx supporting clients in a walk-in clinic setting by providing trauma-informed case management, referrals, and housing-related support in a fast-paced, collaborative environment. About the
Opportunity:
Schedule:
Monday to
Friday Hours:
9am to 5pm (with a 1-hour lunch)
Setting:
Walk-In Clinic (at least 4 days onsite)
Responsibilities:
Provide intensive case management services to walk-in clients in a domestic violence and gender-based violence setting Conduct intakes, assess client needs, and develop appropriate service plans and referrals Support clients with safety planning, crisis intervention, housing guidance, and emergency shelter placements Maintain accurate case notes, digital records, and caseload documentation Collaborate with onsite partners and internal team members to coordinate client-centered services and support high-risk cases
Qualifications:
5+ years of relevant Case Management experience Associate's Degree Experience working with survivors of domestic violence and/or gender-based violence Experience assisting clients with public benefits, housing, Medicaid, SNAP, or similar supports Strong client services background with excellent communication, listening, writing, and documentation skills High emotional intelligence, discretion with confidential information, and the ability to manage multiple priorities in a fast-paced environment
Desired Skills:
Bachelor's degree in Social Work, Psychology, or a related field Experience with trauma-informed, survivor-centered, or shelter-based services Familiarity with working in a Family Justice Center or similar collaborative service environment Working knowledge of Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook Strong relationship-building skills and the ability to work effectively with diverse, underserved populations
