Patient Care Coordinator
Job Description
Minneapolis Radiology Associates (MRA) and Minneapolis Vascular Physicians (MVP) are growing medical and surgical practices in the Northwest metro area. We are seeking a dedicated, compassionate, and detail-oriented Patient Care Coordinator to join our team. The Patient Care Coordinator plays an essential role in guiding vascular patients through their care journey by coordinating appointments, diagnostic testing, surgical procedures, and follow-up care. This position serves as a key point of communication between patients, vascular surgeons, advanced practice providers, imaging departments, hospitals, and other members of the healthcare team. The ideal candidate is organized, dependable, and committed to providing exceptional patient service while managing multiple priorities in a fast-paced healthcare environment. Key Responsibilities Patient Care Coordination Coordinate patient appointments, diagnostic testing, vascular procedures, surgeries, and follow-up care as directed by vascular surgeons and other appropriate clinical providers. Schedule and coordinate CT, MRI, vascular ultrasound, and other ordered imaging studies across multiple locations and healthcare organizations. Coordinate vascular surgeries, angiogram procedures, related appointments, and pre-procedure requirements. Track outstanding appointments, test results, reports, and other components of the patient's established plan of care. Follow up on incomplete or unavailable information to ensure patients receive timely and coordinated care. Assist with chart preparation, medical record retrieval, and documentation needed for patient appointments and procedures. Scheduling & Administrative Support Manage physician and advanced practice provider schedules, including office visits, urgent appointments, cancellations, and schedule changes. Coordinate scheduling across multiple systems and locations while maintaining accuracy and attention to detail. Provide backup coverage for physician vacation schedules and on-call schedules. Process incoming and outgoing phone calls, faxes, mail, and other correspondence. Complete data entry and maintain accurate patient, scheduling, and care coordination information. Respond to patient inquiries and provide timely, professional communication regarding appointments, procedures, and follow-up details. Route clinical questions and concerns to the appropriate licensed clinical staff. Collaborate with physicians, imaging departments, hospitals, clinics, and outside organizations to resolve scheduling barriers and obtain missing information. Team & Clinic Support Provide dedicated coordination support to assigned vascular surgeons and advanced practice providers. Support patient and provider needs across multiple clinic locations. Provide on-site clinic coverage as needed in Robbinsdale, Plymouth, Maple Grove, and Monticello. Adapt to patient, provider, and operational changes throughout the day. Work collaboratively with team members to ensure efficient patient flow and high-quality service. Qualifications & Requirements High school diploma or equivalent required. At least five years of experience in healthcare scheduling, care coordination, medical office support, or a related field preferred. Knowledge of medical and surgical terminology. Experience supporting physicians or coordinating specialty patient care preferred. Experience with electronic health records (EHR) and scheduling systems preferred. Strong verbal and written communication skills. Excellent customer service, administrative, and organizational skills. Ability to manage multiple priorities and navigate various scheduling systems. Strong attention to detail, time management, and follow-through. Sound problem-solving skills and the ability to work independently while escalating clinical questions appropriately. Ability to learn and navigate electronic health records, scheduling platforms, and other healthcare applications. Ability to maintain professionalism, confidentiality, and compassion when working with patients and healthcare providers. Schedule & Work Arrangement Monday through Friday. Daily hours generally fall between 7:00 a.m. and 4:30 p.m. Flexibility is required to meet patient, provider, clinic, and departmental needs. Work-from-home options may be available on non-clinic days after successful completion of training and demonstration of independent performance, consistent with organizational policies and operational needs. On-site clinic coverage may be required at Robbinsdale, Plymouth, Maple Grove, and Monticello.
