Job Description

VNA Health Group is looking for a dedicated and organized Patient Service Coordinator Per Diem to join our Extended Hours Homecare team! In this key role, you will help support our mission by ensuring patients receive the care they need—on time and with compassion. The Patient Service Coordinator will be responsible for coordinating multiple teams with the Extended Hour's Managers to assure appropriate patient assignments on weekends and holidays by the Extended Hours Staff. This position has flexibility with hours.
Principal Responsibilities:
Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers Coordinates and arranges visits with weekend field staff as indicated by the Extended hours Manager Assigns all visits as directed on team reports and communicates outcomes at the end of the shift Responds to the call center agents messages and refers clinical questions to the Extended Hours Triage Manager Adheres to the organization's policy in regards to absenteeism and appearance
Job Competencies & Minimum Qualifications:
High School Diploma/GED required; Associate or Bachelor's degree a plus 2-3 years' office experience required; medical background preferred Strong computer skills and proficient in MS Office Suite Strong organizational skills Strong customer service and interpersonal skills
Working Conditions/Physical Demand:
Hybrid position working from home with rotation in a Business Office Environment. Heavy phone and computer use with high demand of good communication at a steady pace. In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $16.00 to USD $24.00.
