Job Description

Schedule:
Full-time, about 40 hours per week, Monday through Friday days, with some remote work. Also open to two part-time hires (16 to 20 hours each), one per region.
Duration:
Temporary, early November 2026 through late February 2027, with possible extension Reports to: Regional Director of Operations Location and travel: Home base is Neenah, WI, covering our Neenah and Irish Road buildings. If hired as the full-time regional role, you will also travel to our Pewaukee and Hartland buildings in Waukesha County approximately two days per week; mileage for travel between regions is reimbursed at the IRS standard rate. If hired part-time for the Fox Valley only, there is no regional travel. Similar titles : Assisted Living Administrator, Executive Director, CBRF Administrator, Regional Nurse, Wellness Director, Wellness Coordinator, Health Services Coordinator, Resident Care Coordinator, Clinical Coordinator. Requirements Two or more years in a Wisconsin CBRF or assisted living community as an administrator or executive director, resident care coordinator, LPN, RN, or a comparable clinical or compliance role. Working knowledge of Wisconsin DHS 83: incident reports and self-reports, resident assessments and individual service plans (ISPs), and medication management rules. Experience with an assisted living EHR, fax portals and provider portals; able to learn new software fast. Organized and self-directed; able to run a daily audit routine across several buildings without being prompted. Able to pass a Wisconsin caregiver background check.
Preferred :
Active Wisconsin LPN license. Completed CBRF administrator training. Experience with ALIS. The role Adava Care operates licensed CBRFs (community based residential facilities) in the Milwaukee metro, Waukesha County and the Fox Valley. Our regional clinical support lead, who backs up the administrators at our Neenah, Irish Road, Pewaukee and Hartland assisted living buildings, is going on leave. We need an experienced assisted living professional to hold that seat for about three months. This is a follow-through and compliance role with no direct reports. You work alongside the building administrators and their resident care coordinators, keeping the clinical and regulatory work moving: incident report follow-up, resident assessments and ISP updates, medication issues, provider and pharmacy communication, and the monthly and quarterly reviews DHS 83 requires. A nursing license is welcome but not required. What is required is that you already know how a Wisconsin CBRF runs and can walk into someone else's system and operate it. You will get a one to two week overlap with the person you're covering, a written daily routine, and reporting that is already automated. Your time goes to the follow-up and judgment calls, not to building the process. What you will do Daily (remote or on site) Review the medication exception, compliance document and incident report audits for all assigned buildings in our EHR (ALIS) and act on anything requiring follow-up. Review resident charting notes for all assigned buildings.
Work the online fax portal:
send orders and clarifications to the pharmacy, route provider responses, close open items. Review the provider portal (Bluestone) and respond to provider communication.
Follow up on incident reports:
check in with the resident, enter a follow-up note, complete the matching assessment (fall risk, wander/elopement, choking) and update the ISP if needed. When on site (about two days per week in each region) Review the paper charting binder and the pending binder for that building; initial and file returned faxes and documents; resend anything unanswered. Complete new move-in assessments within required timeframes (pain assessment within 7 days; self-administration and smoking assessments where applicable). Complete the weekly narcotic audit (key exchange, count records, dispensing patterns). Review hospital discharge paperwork with the administrator before a resident returns. Assist staff during incidents that occur while you are in the building. Monthly and quarterly First business day of the month: complete the PRN psychotropic review for the prior month and send the monthly reminder to administrators and resident care coordinators (vitals, fresh charting sheets, updated MARs).
First through the seventh:
monitor that monthly vitals are completed across assigned buildings.
December:
prepare the quarterly scheduled psychotropic review for submission to the provider. Confirm ordered labs are completed. What you will not do Supervise, schedule or discipline building staff. Administrators own their buildings. Sign reviews or assessments that DHS 83 assigns to a physician, pharmacist or registered nurse. Those go to the provider or pharmacy. Administer medications or treatments that require a nursing license. Compensation details $33.00 to $38.00 per hour, paid biweekly. Rate set by experience and credentials. W-2 employment with Adava Care. This is not a 1099 or staffing agency position. Mileage reimbursed at the IRS standard rate for travel between regions. Regular commuting to your home-base region is not reimbursed. Temporary position; benefits are not included.

$33.00 - $38.00 per hour Expected hours: 30.0 - 40.0 per week
