The role ensures high-quality resident care, regulatory compliance, and consistent clinical standards. This position provides hands-on leadership, mentors nursing teams, evaluates care systems, and drives continuous improvement through data and oversight. Core Responsibilities Clinical Oversight and Quality Evaluate clinical services, documentation, assessments, care plans, and care delivery Conduct proactive QA audits to identify risk, trends, and compliance gaps Analyze clinical and operational data to identify barriers and improvement opportunities Oversee implementation of clinical initiatives and specialty programs Maintain current, compliant clinical policies and procedures Survey and Regulatory Compliance Lead survey readiness through mock surveys, targeted reviews, and real-time coaching Oversee all survey activity, including preparation, execution, and follow-up Manage EDIC and CLASS access, monitoring, and submissions Guide risk investigations and ensure timely Plans of Correction and follow-through Ensure consistent, ongoing survey readiness across all assigned facilities Leadership and Team Development Support and mentor DONs and nursing leadership Strengthen systems for QA, staffing, infection control, and compliance Promote accountability, quality outcomes, and professional growth Support leadership performance expectations and team structure Infection Prevention Strengthen infection prevention programs across facilities Mentor and monitor infection control staff Ensure regulatory compliance and best practices Facility Presence and Communication Conduct regular facility visits to observe care and validate performance Provide hands-on education and real-time coaching Communicate findings, risks, and action steps clearly to Administrators, DONs, and regional leadership Attend OHCA meetings and represent the organization as needed Additional Responsibilities Perform other duties as assigned by clinical leadership Qualifications Licensure and Education Active Ohio RN license BSN preferred Experience 3 to 5 years of long-term care leadership experience, DON or higher Proven success with SNF survey preparation and outcomes Strong knowledge of state and federal long-term care regulations Skills and Competencies Strong analytical and systems-thinking skills Ability to collect, interpret, and act on data Excellent communication, judgment, and interpersonal skills Ability to manage multiple priorities in fast-paced environments Demonstrated ability to mentor, coach, and build strong nursing teams Other Requirements Valid driver's license Ability to travel between facilities Overnight travel may be required Work Environment Primarily facility-based in the Toledo area, with potential expansion Not an office-based role Limited remote work when operationally appropriate Schedule may shift based on surveys and clinical priorities Regular use of computers, phones, and standard office equipment Compensation and Benefits Competitive salary Paid time off Medical, dental, vision, and life insurance options
