This leadership role ensures the delivery of high-quality, client-centered services that promote recovery, independence, and overall well-being. The Supervisor of Case Management Services will lead a team of case managers, collaborate with internal and external stakeholders, and ensure compliance with regulatory standards and organizational policies.
Scope of Leadership:
Program Management:
Execute strategic plans for the Horizon's Case Management team ensuring they meet the needs of clients and align with best practices. Evaluate program effectiveness and make data-driven decisions to improve services.Team Management:
Recruit, train, and supervise an established and growing team of case managers Provide ongoing professional development opportunities and performance evaluations Foster a positive and collaborative work environment Coach team members to optimal performance Work to engage employees and facilitate employee retentionJob Responsibilities:

Ensure that case management services are client-centered, comprehensive, and culturally competent Develop and implement policies and procedures to enhance service delivery and client outcomes Oversee the assessment, planning, coordination, and monitoring of client services Interview and onboard highly talented providers Communicate and engage case managers in key care management initiatives by sharing a clear, compelling vision that allows them to see their contribution and role in the work Identify and address gaps in service delivery Carry a part time case management caseload of 14/15 clients.Compliance and Quality Assurance:
Ensure compliance with all federal, state, and local regulations, and organizational policies Develop and maintain quality assurance processes to monitor and evaluate service delivery Prepare and present reports on program performance and outcomes Manage case manager performance - includes, but is not limited to: Communication expectations Conducting timely in person performance reviews Coaching Developing skills in documentation, time management, organization Crisis intervention and safety planning Providing recognition Providing disciplinary actions when needed Using client experience data to celebrate and enhance provider practice Accountability for quality outcomes associated within your scope of leadership Manage case manager productivity and address issues in a timely manner Parter with Director of Case Management, Managers, Dakota County contacts, MMHC leadership, and other key stakeholders to ensure adherence to all safety, risk and compliance policies and procedures Provide regular in person presence at appropriate MMHC clinics demonstrating awareness and involvement in program activities.Collaboration and Advocacy:
Advocate for the needs of clients and the importance of case management services at local and national levels Represent the organization at meetings, conferences, and public events Establish and maintain relationships with community partners, healthcare providers, and other stakeholders Establish and maintain relationships with community partners, healthcare providers, and other stakeholdersQualifications:
Education and Experience:
Master's degree in social work is preferred. Bachelor's degree in social work, psychology, or related field with relevant experience will be considered. A minimum of 5 years of experience in case management or related services, with at least 2 years in a supervisory or leadership role is preferred. Relevant experience will be considered. Experience working with adults with serious persistent mental illness. Current active license in the state of Minnesota if BSW or MSW graduate. Good standing with the professional board for your licensure if applicable. Computer skills including using Microsoft Office products and healthcare software.Skills and Competencies:
Strong leadership, team management, analytical, and problem-solving skills. Accomplished organizational and multitasking skills, with the ability to manage multiple projects simultaneously. Excellent communication and interpersonal abilities. In-depth knowledge of case management principles, practices, documentation, and standards. Ability to execute strategic plans and program evaluations. Proficiency in navigating county and state resources. Strong organizational and problem-solving skills. Ability to work collaboratively with diverse stakeholders and advocate effectively.Physical Requirements:
Ability to perform tasks that may involve sitting, standing, and walking for extended periods Occasional lifting of materials or equipment up to 25 poundsWork Environment:
Hybrid home or in-office setting with occasional travel to meetings, conferences, and other sites including client homes, hospitals, clinics, other agencies, and courtrooms This position requires frequent driving including transporting clients on occasion This position is full time, Monday thru Friday 2026Goals:
Work with Director of Horizon's Case Management to create & implement best practices for Dakota County TCM. Obtain team billing rate of 98% Establish a process for quarterly rounding Conduct stay interviews with all case managers Establish meeting structure that supports engagement and growth Improve service processes and procedures where able Help create 30-60-90 day core competencies and onboarding process to ensure the highest quality training and onboarding for our case managers Help write annual reviewsYour Skills & Expertise:
