Athena Hospice of Rhode Island is seeking a dedicated and compassionate Assistant Clinical Director/Case Manager. As an Assistant Clinical Director, you will play a vital role in overseeing the direction of our hospice clinical services. You will help establish, implement, and evaluate goals and objectives for hospice services while also conducting patient visits, admissions, and care planning. Your expertise and leadership will help shape the future of our organization and make a meaningful impact on the lives of our patients and families. 32 hours a week. This is a salaried role with weekly pay ranging from $45 to $50 an hour, depending on experience. Mileage reimbursement provided. 8a-4:30p shift. We service the entire state of Rhode Island. Must be an RN.
Responsibilities:
Oversee all direct and indirect patient services provided by clinical staff Provide clinical oversight, mentoring, and guidance to interdisciplinary team members Assist with hiring, evaluation, and supervision of clinical personnel Develop and maintain clinical policies and procedures that comply with federal and state regulations Provide orientation and ongoing training to clinical staff; oversee in-service education Maintain quality improvement initiatives, monitor performance indicators, and ensure compliance with the performance improvement program Communicate effectively with families, caregivers, and providers regarding goals of care and clinical updates Participate in IDT meetings and care conferences
Requirements:
Must possess an active and unencumbered RN license in Rhode Island Bachelor's degree in nursing At least two years in hospice or home health required
