Position Inpatient Care Coordinator Location Beaufort Memorial Hospital Full-Time/Part-Time Full-Time Shift Days Exempt/Non-Exempt Non-Exempt Description The Care Coordinator is responsible for providing comprehensive case management services to patients with complex medical conditions and/or psychosocial situations to achieve quality cost-effective and optimal clinical outcomes in both inpatient and outpatient settings. He/she will function as an integral member of a collaborative and interdisciplinary team to assist with identifying high risk patients and improving outcomes. Care Coordinator will complete assessments, design/implement care plans, provide education on disease process/management, facilitate referrals to community resources, and provide support and advocacyPosition Requirements Education:
Associate's degree or license in Social Work, Counseling, Nursing (RN), or similar field required. Completion of a Practical Nursing Program (LPN) is also acceptable. Bachelor's or Master's degree preferred.Experience:
At least one year of experience in a healthcare setting and/or acute care; and/or case management experience required. Minimum one year of experience in Behavioral/Mental Health for positions in Behavioral Health areas.Certifications/Licenses:
