About Christ Community Health Services Since 1995, Christ Community Health Services (CCHS) has provided high-quality healthcare to the underserved in the context of distinctively Christian service. As a faith based Federally Qualified Health Center (FQHC), we serve approximately 60,000 patients each year across our Memphis health centers, offering primary care, dental, behavioral health, pharmacy, and specialty services to the communities that need them most. About the Role The Referral Coordinator assists the care team by coordinating care for scheduled patients and proactively organizing care for patients not on the schedule, offering complete preventive care to every patient enrolled
- especially those managing serious, complex, or chronic health conditions or psychosocial needs. The Referral Coordinator requests results for care patients receive outside of CCHS to keep medical records current, ensures patient notes are scanned into the chart and reviewed by a provider, and disseminates data to keep referral workflows moving smoothly across CCHS.
