JOB DESCRIPTION
SUMMARYOF POSITION
The Clinical Care Coordinator is responsible for managing a caseload of clients, documentation and charting in electronic health record, communicating with multidisciplinary teams, educating community on available programming, and ensuring adequate patient care, with a main focus on veterans, military and their families receiving quality outpatient counseling services. They also review recommendations from the patient's mental health care providers and coordinate treatment and community resources available to the patient through the Military Services program and partnered organizations. Military cultural competency and a well-developed understanding of community resources for outpatient counseling is crucial. The care coordinator should be engaging and energetic while maintaining professional relationships with the community they will serve and are capable of engaging the Veteran and civilian community using a variety of modalities including online, virtual, and in- person means.
ESSENTIAL DUTIES & RESPONSIBILITIES HIPAA
Compliance:
Perform all duties within HIPAA regulations.Policy Implementation:
Implement policies and procedures across the enterprise.Care Coordination:
Engage and link referrals for outpatient counseling services with providers across the US and abroad using shared-decision making model to ensure patient care meets individualized needs and program contractual standards.Patient Safety:
Conducting follow-up calls to assess risk level for individuals who have indicated history of suicidal ideation, provide safety management as necessary, and deliver timely referrals for servicesCredentialing:
Contribute to the management of resource database and provider network credentialingAdmissions and Treatment Authorization:
Authorize outpatient treatment services as per program specific protocols.Communication:
facilitate communication and collaboration among diverse community settings as well as across both internal and external teams involved in patient's care. Demonstrate adaptability and active engagement as a team playerOutreach:
Increase positive awareness of Centerstone Military Services in diverse community settings and the mission to empower veterans and their families to thrive; Connect with local and national organizations with complementary and similar resources to help shape specific Centerstone programs.Engagement:
Drive eligible patients and their families to utilize Centerstone's programs while also identifying and addressing barriers to engagement.Program Development:
Assess programs to determine efficacy and areas of improvement.KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods. Strong communication skills, both written and verbal Punctual and dependable with meetings, deadlines, and administrative task support High level of customer service skills and professional etiquette and conduct professional follow ups with leadership, patient base, and stakeholders. Ability to work both independently and as an engaged member of a team through attendance at weekly team huddles, monthly team syncs, and 1x1 supervision. Familiar with current social media platforms. Excellent analytical and organizational skills. Ability to problem solve complex issues and deliver effective decision making. Effectively communicate via written, verbal, in person and virtual methods. Demonstrate knowledge and competency in providing service linkage and referrals for programs, services, and other community resources. Knowledge of suicide assessments and interventions Therapeutic skills and knowledge of EBTs. Working knowledge of Microsoft Office [specifically Excel], Adobe, and Zoom software applications. Excellent analytical and organizational skills. Ability to problem solve complex issues and deliver effective decision making. Demonstrate knowledge and competency in providing service linkage and referrals for programs, services, and other community resources. Knowledge of suicide assessments and interventionsQUALIFICATIONS
Education Level Bachelor's Degree in Social work, Psychology or other related field is requiredYears of Experience Preferred Experience:
Must have serviced in the military, or is a spouse of an active-duty service member or veteran, Gold Star spouses or family members, or is family of a service member Outreach and community referral experience preferred Certification/LicensureN/A PHYSICAL REQUIREMENTS
