Position Summary St. Vincent's House is seeking an experienced and compassionate Masters prepared Director of Case Management to lead our case management team and oversee programs serving individuals and families facing housing instability, homelessness, financial hardship, health challenges, and other complex needs. This is a hands-on leadership position for a seasoned social worker who enjoys both leading a team and working directly with clients. The Director will supervise and develop case management staff, oversee program quality and outcomes, strengthen community partnerships, and personally manage a select caseload of clients with complex or high-acuity needs . Key Responsibilities Lead, supervise, mentor, and develop the case management team. Provide oversight of case management programs, policies, documentation, quality, and client outcomes. Personally, case manage select clients and families with complex, high-risk, or multi-system needs . Conduct assessments and develop individualized service plans focused on stability and self-sufficiency. Coordinate services across housing, healthcare, behavioral health, benefits, transportation, legal services, food assistance, and other community resources. Provide crisis intervention and professional guidance on complex or sensitive cases. Conduct case reviews and support staff in developing solutions for challenging client situations. Ensure programs meet grant requirements, organizational policies, and professional standards. Monitor program data and outcomes and identify opportunities for continuous improvement. Build strong partnerships with healthcare providers, housing organizations, nonprofits, governmental agencies, and other community resources. Represent St. Vincent's House in community collaborations and deliver professional presentations to partners, funders, and other stakeholders as needed. Advocate for the individuals and families we serve.

Pay:

$51,606.34 - $70,000.00 per year
