Job Description
Encompass Health seeks a Patient Assessment Standards Coordinator (RN) to lead clinical assessment quality in our inpatient rehabilitation setting. In this role, you will coordinate and validate patient assessments, ensure accurate documentation, and support compliance with regulatory and reimbursement standards. You will collaborate with an interdisciplinary team to optimize patient outcomes, provide education on assessment protocols, and monitor performance metrics. Responsibilities Coordinate, perform, and validate patient assessments in the inpatient rehabilitation setting Ensure accurate, timely, and complete clinical documentation for regulatory and reimbursement needs Monitor compliance with assessment standards, CMS, and Joint Commission requirements Educate nursing and interdisciplinary staff on assessment protocols and documentation best practices Collaborate with physicians and rehab team members to support optimal patient outcomes Analyze assessment data to identify trends, gaps, and opportunities for quality improvement Participate in development and updates of assessment-related policies and procedures Support survey readiness and internal audits related to clinical documentation and assessments Serve as a clinical resource for complex assessment or documentation questions Contribute to a patient-centered, safety-focused culture aligned with Encompass Health values Required Skills Clinical patient assessment Inpatient rehabilitation nursing Electronic health record (EHR) documentation IRF-PAI and/or OASIS completion Clinical quality improvement Care coordination and interdisciplinary collaboration Regulatory compliance (CMS, Joint Commission) Data analysis and reporting Patient education and discharge planning Policy and procedure development
