Come join our team! We are looking for an intake coordinator who is caring and passionate about providing the best customer service. Home health and hospice Intake Coordinator Our Intake Coordinator is responsible for taking the referral from the initial contact through the admission, interacting with patient, family, healthcare professionals and team members. (Experience desired but will train the right person)

Intake Responsibilities:
Interact well with physicians, discharge planners, patient, family, team members and other healthcare professionals. Perform insurance eligibility and benefits information using phone and online resources. Understanding various aspects of medical insurance, including Medicare. Accurately enter all necessary referral information and documentation, verifying demographics and physician orders in the EMR computer system. Collect all face to face documentation for review. Respond to requests in an accurate and timely manner. Coordinate scheduling with appropriate team members ensuring they receive all necessary information to begin patient care. Provide excellent telephone and customer service. Answer and return telephone calls promptly and courteously. Follow HIPPA requirements to maintain confidentiality.
