Rocky Mountain Care is looking for an experienced Home Health Clinical Director / Director of Clinical Services to lead our Home Health and Hospice team in Utah County. What You'll Lead As Director of Clinical Services, you'll oversee Home Health and Hospice clinical operations and partner directly with the Administrator to drive: Clinical quality and patient outcomes OASIS accuracy and clinical documentation CMS quality measures and 5-Star performance Medicare and regulatory compliance Clinical staff development and accountability Quality improvement initiatives Case management and plan-of-care oversight Staff engagement and retention Interdisciplinary collaboration Strong, consistent clinical processes OASIS Experience Is a Must. We're looking for someone who knows Home Health. You must have strong hands-on experience with OASIS and understand how clinical practice, documentation, patient outcomes, CMS quality measures, and regulatory compliance work together.

You should also bring:

Current RN license in good standing Home Health clinical leadership experience Strong clinical judgment and communication skills Experience with CMS quality measures and 5-Star ratings Ability to thrive in a fast-paced environment A passion for coaching and developing people Hospice experience is preferred, but not required. Requirements:
