The Community Health Coordinator is responsible for advancing community-based substance use prevention, intervention, treatment, and recovery initiatives through strategic coordination, collaboration, and resource development. This position serves as a central point of connection among community partners, service providers, and stakeholders to improve communication, strengthen service systems, and enhance access to resources for individuals and families. The Coordinator supports the effective implementation of programs funded through Opioid Abatement and other available resources by promoting data-informed decision-making, identifying community needs, and fostering partnerships that improve community health outcomes. This position works across multiple sectors to align prevention and intervention efforts, promote evidence-informed practices, and support a coordinated community response to substance use challenges. This position is grant-funded and is anticipated to continue for five years or longer, contingent upon the availability of funding and subject to periodic evaluation based on program needs and funding status. Serve as the primary grant monitor for Opioid Abatement funding and ensure compliance with grant requirements, reporting obligations, and program objectives. Develop, implement, and maintain a data tracking system to document citizen service utilization, outcomes, trends, and program effectiveness. Organize and facilitate regular meetings among human service organizations to review program progress, identify service gaps, and develop coordinated strategies to address community needs. Coordinate local and regional partners engaged in substance use prevention, intervention, treatment, and recovery efforts. Facilitate cross-sector collaboration among community partners, including but not limited to Law Enforcement, Fire and Rescue, Social Services, Horizon Behavioral Health, Bedford County Public Schools, and service providers affiliated with the Bedford Area Resource Council (BARC). Participate in Bedford Area Resource Council (BARC) meetings, committees, and action groups to support coordinated community health initiatives and strengthen partnerships among service providers. Source and provide educational opportunities, training, and outreach activities for County agencies, state partners, community organizations, and other stakeholders. Monitor and publicly report local substance use indicators, overdose trends, and fatalities to inform community priorities, prevention strategies, and program development. Actively pursue and align additional grant opportunities to maximize available funding and expand prevention, intervention, and recovery resources. Assess the needs of individuals and families, including students and caregivers when appropriate; provide short-term case management, service navigation, referrals, and follow-up to support access to needed resources. Collaborate with multidisciplinary teams to develop and implement prevention and intervention strategies that address identified community needs. Monitor service linkages, referral outcomes, and community resource utilization to improve coordination and effectiveness of services. Ensure program activities align with evidence-informed practices, grant requirements, and established community health goals. Supervise additional staff assigned to the Community Health Program as future grant funding allows.
Education / Experience
Bachelor's degree in human services, public health, behavioral health, social work, or a related field, supplemented by professional experience in community health, substance use prevention, behavioral health, or human services program coordination. Experience in program oversight, supervision, grant management, and collaborative partnerships is preferred. An equivalent combination of education, training, and experience that provides the required knowledge, skills, and abilities may be considered. Significant professional experience in a related field is required.
