Develop and update individualized care plans. Complete caregiver supervisory visits and annual performance evaluations. Perform caregiver skills assessments, competency evaluations, and new hire orientation. Caregiver recruitment, interviewing, onboarding, and scheduling. Review clinical documentation for accuracy and compliance. Educate and mentor caregivers on dementia care, infection control, fall prevention, and best practices. Ensure compliance with Virginia home care regulations and agency policies. Collaborate with the Director of Nursing and office team to coordinate quality client care. Serve as a clinical resource for caregivers, clients, and families. Help resolve client and caregiver concerns while promoting excellent customer service. Qualifications Current Virginia Licensed Practical Nurse (LPN) / Register Nurse (RN) license in good standing. Graduate of an accredited practical nursing program. Minimum of one year of nursing experience (home care experience preferred). Current CPR certification. Strong communication, leadership, organizational, and computer skills. Valid driver's license and reliable transportation. Willingness to travel throughout Northern Virginia and Shenandoah Valley for supervisory visits and client assessments. Valid nursing license with current state licensure; LPN (Licensed Practical Nurse)/RN (Registered Nurse) preferred. Extensive experience in home care settings with proficiency in medical-surgical nursing, long-term care regulations, and acute care experience. Demonstrated supervisory or nurse management experience leading multidisciplinary teams within home health or hospital environments. Expertise in managing complex cases involving ventilator management, IV infusion, catheterization, tube feeding, wound care (including suturing), and infection control protocols. Familiarity with healthcare procedures including vital signs assessment, medication administration, phlebotomy, specimen collection/processing, and sterile technique. Knowledge of healthcare regulations including HIPAA compliance, workers' compensation law, long-term care regulations, and discharge planning requirements. Excellent leadership skills with the ability to manage teams effectively while maintaining high standards of patient safety and satisfaction.
