Job Description

Position Summary We are seeking a highly organized, motivated, and customer-focused Home Health Coordinator to join our growing team. This is a non-clinical, in-person position that plays an essential role in supporting our clinicians, patients, families, and referral partners. The ideal candidate is eager to learn, thrives in a fast-paced environment, and can effectively balance multiple priorities while maintaining exceptional attention to detail. This position requires excellent communication skills, strong organizational abilities, and a passion for providing outstanding customer service. The successful candidate will be comfortable working both independently and collaboratively while helping ensure the smooth day-to-day operation of our home health services. Essential Responsibilities Coordinate and manage physician orders, documentation, and order tracking. Assist with patient scheduling and coordination of clinician visits. Maintain communication with patients, families, referral sources, physicians' offices, and other healthcare partners. Coordinate patient supply orders and ensure timely delivery of necessary equipment and supplies. Support the clinical team with administrative tasks to promote efficient patient care. Answer incoming phone calls professionally and provide exceptional customer service. Serve as a welcoming first point of contact for clients and visitors. Assist with maintaining accurate records and ensuring timely follow-up on outstanding items. Work collaboratively with nurses and office staff to prioritize daily workflow and meet organizational goals. Participate in quality improvement initiatives and process optimization. Assist with office operations and other administrative duties as assigned. Marketing Responsibilities Marketing is an important component of this position. The Home Health Coordinator will actively support the organization's growth by: Building and maintaining professional relationships with referral sources and community partners. Conducting both remote outreach (phone, email, social media engagement) and in-person marketing visits. Representing the organization professionally within the community. Collaborating with the marketing team to implement outreach initiatives and community engagement efforts. Assisting with promotional events, networking opportunities, and community education activities. Qualifications Preferred Previous medical office, home health, or healthcare administrative experience. Front desk or receptionist experience. Customer service experience in a professional setting. Experience with scheduling, medical records, or healthcare documentation. Skills & Attributes Excellent verbal and written communication skills. Strong organizational and time management abilities. Ability to prioritize multiple tasks while meeting deadlines. Professional, compassionate, and patient-focused demeanor. Comfortable communicating with patients, families, physicians, and referral sources. Self-motivated with the ability to work independently while contributing positively to a team. Strong computer skills and willingness to learn new software systems. Adaptable, dependable, and eager to grow professionally. Positive attitude with excellent problem-solving skills. Reliable transportation for occasional in-person marketing visits and community outreach. Pay:
