POSITION SUMMARY
The Patient Care Coordinator manages referrals, authorizations, potential scheduling leads, and follow-up care, reducing referral leakage, delays, and denials. By coordinating non-clinical needs, the PCC protects physician and MA capacity, supporting increased patient volume and revenue.
SCHEDULE
This is a full-time position working Monday through Friday, 8:00am - 5:00pm.
ESSENTIAL FUNCTIONS
Schedules consultation and diagnostic appointments for patients per provider order. Completes requested forms. Obtains insurance authorization for appointments. Contacts patients to provide appointment time, location and preparation information for appointment. Answers patient questions regarding appointments and testing. Maintains ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. Ensures complete and accurate registration, including patient demographics and current insurance information. Functions as the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. Ensures that referrals are addressed in a timely fashion. Functions as back up to Patient Service Representatives performing a variety of office assistant related functions. Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise. Ensures no injuries to self or others by following safe work practices and policies. This includes, but is not limited to: security and safety, understanding of chemical Safety Data Sheets (SDS), equipment, infection control, fire, disaster, safe lifting and body mechanics. Ensures self-compliance with organization policies and procedures, as well as labor agreements. Ensures the interface with team members and other support groups is conducted in a courteous and efficient manner conducive with the organization's values. Conducts self in a professional manner and ensures personal appearance meets the standards necessary to perform the job function while representing the organization. Ensures that additional accountabilities, as may be required by management, be handled in a manner necessary to meet organizational standards.
EDUCATION & EXPERIENCE
Education:
Associates degree; experience will be considered in lieu of education.
Experience :
Minimum of one (1) year of medical office experience preferred. Experience in a hospital or physician office with a background in medical terminology, insurance, CPT codes or ICD- codes preferred.
Skills/Competencies:
Medical terminology, CPT and ICD-9 code knowledge. Demonstrates competency on equipment listed on department specific checklist.

Exercises safe judgment in decision-making. Practices within legal and ethical guidelines. Demonstrates competency in ability to care for customers/patients across the age continuum. Current (Healthcare Provider [HCP] level) CPR certification or ability to complete within three months of hire.
PHYSICAL REQUIREMENTS
Occasional lifting, reaching, kneeling, bending, stooping, pushing and pulling. Occasional heavy lifting (lift/carry up to 50 lbs.). Manual dexterity and mobility. Ability to read and understand patient charts, provider orders, tests results, etc. Ability to communicate using verbal and/or written skills for accurate exchange of information with physicians, nurses, health care professionals, patients and/or family, and the public.
