Credentials:
Active MA, LPN, or RN license/certification in the state of
Hawaii Technical:
Computer savvy; intermediate proficiency in MS Suite —

WHAT YOU'LL DO
Maintain care coordination dashboards, patient lists, and tracking for CCM/PCM programs Track referrals, authorizations, and follow-up appointments across specialty and post-acute services Conduct non-clinical patient and family outreach, including welcome calls and appointment reminders Coordinate communication between patients, facilities, Care Managers, and providers Gather, upload, and organize medical records, wound photos, labs, and imaging reports Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments Complete clerical tasks related to Care Management, including outreach, scheduling, and documentation Monitor admissions, discharges, and census changes from all care settings Prepare patient summaries, reports, and materials for care team meetings Escalate clinical concerns promptly to a Care Manager or provider Participate in required training, meetings, and compliance activities
