    Rapid Patient Triage:
    Perform swift, accurate triage evaluations (in-person or via telephone) to assess injury or illness severity, prioritizing care based on patient acuity and immediate needs.
    Clinical Assessments:
    Conduct targeted physical examinations, capture comprehensive patient histories, and continuously track vital signs (e.g., heart rate, blood pressure, pulse oximetry).
    Minor Procedures Support:
    Prepare for and assist healthcare providers with minor office procedures such as suturing, incision and drainage (I&D), wound care, and splinting.
    Interventions & Wound Care:
    Perform phlebotomy and sample collection, administer prescribed oral, injectable, and provide complex wound care or dressing changes.
    Emergency Response:
    Recognize signs of physiological deterioration or life-threatening emergencies and immediately initiate basic life support (BLS) interventions while coordinating rapid transfer to a higher tier of care.
    Patient Flow Management:
    Monitor and drive clinic workflows, collaborating closely with providers, medical assistants, and front-desk personnel to reduce waiting times and ensure room availability.
    Results & Communication Management:
    Review and act upon incoming laboratory, point-of-care, and radiology results; flag critical findings for immediate provider review and communicate results to patients.
    EHR Documentation:
    Maintain exact, comprehensive, and legally compliant medical records within the Electronic Health Record (EHR) system for all clinical encounters, phone calls, and treatments.
    Patient Advocacy & Care Continuum:
    Assist in coordinating referrals, outpatient imaging, or direct hospital admissions, ensuring seamless transitions across the care continuum.
    Education & Teaching:
    Educate patients and caregivers regarding their final diagnoses, prescribed medication regimens, home care protocols, and necessary follow-up instructions.
    Staff Support & Mentorship:
    Serve as a clinical resource and mentor to Licensed Practical Nurses (LPNs), Medical Assistants (MAs), and other ancillary clinical support staff.
    Quality & Safety Compliance:
    Actively participate in facility quality improvement initiatives, infection prevention, compliance audits, and equipment sterilization checks to uphold strict safety standards.
    • Provides culturally and clinically appropriate medical and mental health education and information to patients and coaches patients in making healthy lifestyle changes with direction of PCP
    • Acts as the patient's advocate and health navigator, assisting them in understanding the most appropriate use of health care resources.
    • Works with the insurances and outreach community members to gather and maintain information, resources and accessible services of community agencies, local and state organizations, acting as a liaison with outside agencies.
    • Builds a supportive relationship with patients to improve patients' utilization of necessary and appropriate primary care, mental health, and social services, in order to improve the patients' health status and health outcomes
    • Uses motivational interviewing to assess and positively impact the patient's level of engagement in his or her care and confidence in his or her ability to carry out a self-management plan.
    • Participates with the team in developing a comprehensive care plan.
    • Will also work with the Primary Care Teams to outreach to patients with complex health and/or psychosocial needs and those who are high utilizers of hospital and Emergency Department services to remove barriers to care.
    • May facilitate, coordinate and organize group education or group provider visits, if approved by the Clinic Manager in consultation with the site management team.
    • Maintains timely, detailed and accurate documentation of all encounters and patient accounts
    • Attend regular training as required
    • Performs other duties as assigned
    Skills:

    • Two years' working experience in ED or health-related field preferred
    • Prior experience in RN Care or Case Management
    • Training in Motivational Interviewing preferred
    • Grammar, spelling, and punctuation
    • Ability to work in fast paced and challenging environments
    • Experience with group facilitation, ability to work as an integral part of a high functioning team
    • Strong user of Microsoft Word and Excel. Prior EMR experience preferred
    • Excellent Customer Service skills
    • Current CPR/BLS certification
    Experience:
    2-year clinical experience preferred.
    QUALIFICATIONS
    & requirements:
    Education/ Certification/Licensure:
    Education:
    Graduate of an accredited School of Nursing (Associate Degree in Nursing, ADN, or Bachelor of Science in Nursing, BSN).
    Licensure:
    Active and maintained, unencumbered Registered Nurse (RN) license in the state of practice.
    Certifications:
    Current American Heart Association Basic Life Support (BLS) provider certification. Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) is highly preferred.
    Technical Skills:
    Competency utilizing Electronic Health Record (EHR) software (e.g., Epic) and administering point-of-care testing. Previous experience with project management Strong organizational and interpersonal skills. Two years' experience in ED Care or Two years as an RN in a clinical setting Ability to multi-task efficiently and effectively. Must be able to act calmly and effectively in a busy or stressful situation. Ability to communicate and document clearly, and effectively in the English language in person, by phone and in writing. Requires adherence to all policies and procedures, including but not limited to standards for safety, attendance, punctuality and personal appearance.
    Physical Requirements:
    Hearing:
    Adequate to perform job duties in person and over the telephone. Able to use stethoscope and respond to verbal questions.
    Speaking:
    Must be able to communicate clearly to patients in person and over the telephone.
    Vision:
    Visual acuity adequate to perform job duties, including visual examination of patient and reading information from printed sources and computer screens.
    Other:
    Requires frequent lifting and carrying items weighing up to 30 pounds unassisted, including assisting patients when required. Adequate physical ability includes sufficient manual dexterity to perform the requisite health care service, including injections. Requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift. Requires exposure to communicable diseases, bodily fluids, medicinal preparations and other conditions common to a clinic environment.
