Job Description
Bluffview Memory Care provides assisted living services for residents with Alzheimer's or other forms of Dementia. We're seeking an experienced Healthcare professional for our Resident Health Coordinator opening. The Resident Health Coordinator is accountable for ensuring the health, safety, and wellness of our residents.
Essential functions include:
- Resident Assessments
- Individual service plan development, implementation, and review
- Evaluation of resident evacuation limitations
- Assisting in the supervisory functions, activities, and training of Resident Care Coordinators.
- Time Management
- On-Call Availability
- Regularly communicates, advises, and educates direct care staff
- Monitor and regulate Medication Administration and Management
- Coordinates health service with other departments.
- Assist with developing and maintaining resident care policies and procedures
- Maintaining documentation standards
- Daily rounding and compliance audits
- Attends administrative staff meetings, participates as required in committees
- Answers inquiries by residents or families as required
- Participates in planning and budgeting for health services
- Develops and periodically updates objectives that define the type of health care the facility provides
- Has thorough knowledge of and enforces DHS 83 regulations and all other requirements.
- Participates in the survey process, responding to any inquiries by regulatory agencies and consulting agencies to provide information and help as required
- Acts as representative of the facility in the community or at professional meetings This is a managerial position that requires oversight and monitoring of caregiving staff.
