St. Camillus is seeking an experienced Hospice Clinical Car Manager to coordinate the day-to-day operations of our Hospice Agency and help ensure the delivery of quality, comprehensive care in accordance with federal, state, Joint Commission, and St. Camillus guidelines.
In this role you will:
Lead, coach, and support Hospice clinical staff. Coordinate admissions, daily nursing visits, case conferences, and care services. Monitor quality outcomes, documentation, compliance, and reporting practices. Serve as a resource and liaison for staff, physicians, allied health professionals, families, and campus partners. Support budgeting, productivity standards, staff development, and ongoing training.
What we are looking for:
Current Wisconsin RN license and current CPR certification. Minimum of five years of hospice experience. At least two years of supervisory experience, including coaching, evaluating performance, disciplinary action, and quality improvement. Strong knowledge of hospice guidelines, regulations, coding, and reporting practices. Excellent communication, presentation, organization, and leadership skills. Valid driver's license, and a driving record that meets St. Camillus Safe Driving Policy requirements. Benefits of working at St.
