The ideal candidate will have experience in various aspects of caregiving, particularly in assisted living and day program services working with individuals with developmental disabilities. This role is pivotal in ensuring that our clients receive the highest quality of care while promoting their independence and dignity. As a Service Manager, you will be responsible for developing and implementing individualized service plans with the Persons served, managing daily activities, supervising direct care staff, and providing essential support to Persons served. Duties Develop and implement comprehensive service plans tailored to individual client needs. Assist individuals with developing independent living skills and activities of Daily Living (ADLs) including bathing, dressing, grooming, and meal preparation. Provide direct support to individuals with developmental disabilities including but not limited to Down's syndrome, intellectual disability, and autism. Ensure compliance with HIPAA regulations and maintain confidentiality of persons served information. Facilitate communication between clients, families, healthcare professionals, and regional centers service coordinators to ensure coordinated care. Conduct regular assessments to evaluate individuals progress and modify service plans as necessary. Provide training and supervision to caregiving staff on best practices in service delivery. Manage conflict situations effectively while maintaining a positive environment for Persons served. Maintain accurate documentation regarding service delivery and care activities. Qualifications Minimum of 3 years of experience in human service delivery is preferred. Familiarity with caregiving techniques for individuals with developmental disabilities. Strong interpersonal skills with the ability to work collaboratively within a team environment. Experience in social work or case management is a plus. Excellent organizational skills with attention to detail in managing care plans. Valid driver's license is required for transportation needs. CPR certification is required.
