Job Summary We are seeking a highly motivated and experienced Designated Coordinator to lead and oversee the operations of our long-term care facility. In this vital role, you will coordinate resident care, ensure compliance with health regulations, and manage staff to deliver exceptional senior and memory care services. Responsibilities Develop, implement, and monitor individualized care plans tailored to residents' specific needs, including Alzheimer's and dementia care. Supervise and support care staff, ensuring adherence to care protocols, medication administration procedures, and safety standards. Oversee daily facility operations, including staffing schedules, budgeting, and resource allocation to maintain high-quality service delivery. Ensure compliance with all health regulation policies, long-term care regulations, and assisted living standards through regular audits and staff training. Maintain accurate documentation related to resident health records, incident reports, and regulatory compliance using Microsoft Office tools. Collaborate with healthcare professionals, families, and external agencies to coordinate comprehensive care services for residents with disabilities or developmental disabilities. Lead efforts in managing financial aspects of the facility such as budgeting, financial management, and healthcare financial oversight. Experience Proven experience working in nursing homes, assisted living facilities, or residential care homes with a focus on senior or memory care. Extensive knowledge of 245D and long-term care regulations and health policy compliance. Demonstrated management experience supervising care teams within healthcare or senior living environments. Strong background in 245D knowledge. Experience working with individuals with developmental disabilities or complex health needs is highly desirable. Previous roles involving operations management or executive leadership in healthcare settings are preferred.
