Working on-site at Rochester's leading hospitals, you'll collaborate with physicians, nurses, case managers, and patients to ensure a smooth discharge process and the successful continuation of care at home. This is an excellent opportunity for healthcare professionals who enjoy building relationships, coordinating patient care, and making a meaningful impact on patient outcomes. What You'll Do Partner with hospital case managers, nurses, physicians, social workers, and discharge planners. Coordinate respiratory and durable medical equipment services for patients transitioning home. Educate patients and caregivers on equipment operation, safety, and maintenance. Complete referral documentation, insurance verification, and patient records. Maintain strong relationships with hospital personnel and referral sources. Support compliance, quality initiatives, and patient satisfaction efforts. Travel locally for patient visits and equipment setup using a company-provided vehicle. Qualifications Required High School Diploma or GED Valid driver's license with a clean driving record Strong communication and organizational skills Ability to work independently in a fast-paced healthcare environment Preferred Hospital, healthcare, or medical office experience Experience with respiratory services, home healthcare, or durable medical equipment (DME) Knowledge of discharge planning, patient coordination, or case management Medical terminology knowledge Experience with insurance verification or medical billing 3+ years of related healthcare experience Why Join Rotech?
