Job Description

Home and Community Support Provider Do you have a passion for helping others and making a real impact? As a Home/Community Provider, you'll work directly with individuals to build life skills, promote independence, and support success in everyday living. Job Summary The Home and Community Support Provider implements individualized habilitation plans for eligible participants while providing direct care in home, community, or center-based settings. This role works closely with families/guardians and the Home/Community Services Manager to support consistent strategies that can be carried over into daily routines. Key Responsibilities Provide direct care based on the participant's Plan of Care (POC), including support with daily living skills such as meal preparation, hygiene, dressing, and recreational activities Support development of self-help, socialization, and adaptive skills Ensure participant health, safety, and well-being at all times Prepare meals or assist participants in meal preparation to meet nutritional needs Observe, supervise, and document participant activities and progress Communicate regularly with supervisors and team members regarding progress and recommended adjustments to care strategies Maintain accurate documentation and complete required reports Respond appropriately to emergencies or request assistance when needed Enter participant information and case notes into the electronic medical record (Therap) in compliance with agency, state, and federal requirements Attend required trainings, staff meetings, and in-service sessions Maintain compliance with all agency policies, procedures, and regulatory requirements Perform other duties as assigned Minimum Qualifications High school diploma or equivalent Experience working with individuals with developmental disabilities preferred, but not required Must meet agency driving requirements (minimum age 21 and licensed for at least 3 years) Valid driver's license and reliable transportation Knowledge, Skills & Abilities Strong communication skills (written and verbal) Ability to work independently and manage time effectively Observational and organizational skills to track participant progress Ability to learn and apply intervention strategies Dependable, punctual, and professional Comfortable using computers and electronic medical record (EMR) systems Ability to learn new systems, including ADP for timekeeping Demonstrated interest in providing direct care and supporting individuals' physical, emotional, and social needs Certifications (Provided by Agency) CPR (Adult/Child/Infant) First Aid Safety-Care Certification Requirements Alaska Driver's License Annual PPD test Hepatitis B immunization or signed waiver Working Conditions Medium work: may require lifting up to 50 pounds occasionally, 20 pounds frequently, and 10 pounds regularly Physical activities include standing, walking, bending, lifting, pushing, pulling, and assisting individuals with mobility Work may take place in home, community, or center-based environments Fairbanks Resource Agency shall abide by the requirements of 41 CFR 60-741.5(a).
