In this vital role, you will provide compassionate, personalized care and support to promote independence, safety, and well-being. Whether working in a group home, assisted living facility, or community setting, you will be a trusted partner in helping residents thrive. This rewarding position offers an opportunity to empower others while developing your skills in patient care, caregiving, and community support. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to enhance their comfort and independence Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals Support medication administration following HIPAA guidelines to ensure safety and compliance Prepare nutritious meals and assist with meal planning to meet individual dietary needs and preferences. Maintain a clean, safe environment by performing regular cleaning tasks and adhering to infection control protocols Document care plans, patient observations, and incident reports accurately to ensure continuity of care Experience Prior experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer's and memory care), or in hospice or home health settings is highly valued Demonstrated knowledge of patient monitoring techniques, vital signs collection, and basic nursing skills such as CPR and first aid certification Experience in group homes, assisted living facilities, nursing homes or long-term care environments is preferred Familiarity with medication administration procedures and care plan implementation is a plus Ability to perform heavy lifting safely and operate equipment like Hoyer lifts or wheelchairs when needed Strong communication skills for working collaboratively with team members, residents, families, and healthcare providers Knowledge of HIPAA regulations to protect resident privacy and confidentiality Join us if you're passionate about providing compassionate support that empowers individuals to live their best lives.
