Job Overview We are seeking a compassionate and dedicated Caregiver/Personal Care Assistant to join our team and provide exceptional support to individuals in need of assistance with daily living activities. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Monitor patients' health status by observing vital signs, mental state, and overall condition; promptly report changes to healthcare professionals. Support medication administration following care plans while adhering to HIPAA regulations and safety protocols. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking. Help with mobility and transfers using proper patient transfer methods and equipment like Hoyer lifts; ensure safety during movement. Provide companionship and emotional support to foster a positive environment; engage clients in conversation and activities. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Assist with transportation for appointments or errands using personal transportation assistance skills. Follow all care plans meticulously, including documentation of patient observations, medication schedules, and progress notes. Ensure residents' rights are respected at all times by promoting autonomy and dignity while adhering to regulatory standards such as HIPAA. Experience Prior experience working in group homes, assisted living facilities, nursing homes or home health settings is highly preferred. Demonstrated experience in patient observation, monitoring vital signs, and implementing care plans. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and supporting individuals with neurodevelopmental or developmental disabilities is advantageous. Familiarity with manual handling techniques including heavy lifting and patient transfer methods like Hoyer lifts. Experience working with individuals with autism or intellectual disabilities supports a versatile caregiving skill set. Strong communication skills for effective client interaction and team collaboration. Certification in CPR/First Aid is required; additional certifications such as HIPAA training or medication administration are a plus.
