Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient health status, including vital signs and overall condition Administer medications and perform catheter care following medical instructions and home healthcare regulations Support patients with memory care needs such as Alzheimer's disease and dementia, ensuring safety and comfort Document patient information accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA standards Facilitate patient transfers using proper methods such as Hoyer lifts or patient transfer techniques Provide companionship and emotional support, especially for geriatric patients or those receiving hospice or end-of-life care Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated ability to work with individuals with disabilities or developmental disabilities Knowledge of medical terminology, medication administration, and medical documentation practices Experience providing dementia or Alzheimer's care, memory care, or hospice support is advantageous Familiarity with home health agency operations, EMR/EHR systems, and home health care regulations Certification in first aid, basic life support (BLS), CPR, and heavy lifting techniques is desirable Proven ability to communicate effectively with clients and team members while maintaining patient confidentiality Join us to deliver compassionate home health care that enriches lives.
