Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature, documenting changes accurately in electronic health record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral support Facilitate safe patient transfers using appropriate patient transfer methods and equipment like Hoyer lifts or transfer belts; assist with mobility and positioning as needed Perform catheter care, wound care, and end-of-life support with compassion and professionalism Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring proper infection control practices Skills Demonstrated experience working with individuals with disabilities, developmental delays, or in assisted living environments Strong knowledge of medical terminology, medical documentation, and use of EMR/EHR systems for accurate record keeping Proficiency in patient care techniques including vital signs measurement, medication administration, catheter care, and CPR/First Aid certification Excellent communication skills to effectively interact with patients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines Ability to perform heavy lifting safely and operate mobility aids like Hoyer lifts or patient transfer devices Experience in geriatrics or long-term care settings with a focus on senior care or hospice services is highly valued Knowledge of home health care assistance regulations and best practices for providing compassionate end-of-life care in a home environment Embark on a rewarding career where your caregiving expertise transforms lives daily.
