Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and document patient health status by observing vital signs such as blood pressure, pulse, respiration, and temperature. Support medication administration following prescribed protocols while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record patient information and medical documentation. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies. Assist with catheter care, wound dressing changes, and other medical procedures under supervision or direction of nursing staff. Facilitate patient transfers using patient transfer methods such as Hoyer lifts or other assistive devices while ensuring safety for both patient and caregiver. Support end-of-life or hospice care with compassion, dignity, and respect for patient wishes. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Maintain clear communication with clients' families and healthcare teams to ensure consistent quality of care and address any concerns promptly. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred. Demonstrated experience working with individuals with developmental disabilities or geriatrics is advantageous. Familiarity with medical terminology, medication administration procedures, and vital signs measurement is essential. Experience working with EMR/EHR systems for accurate documentation of patient records is required. Knowledge of home health care regulations, HIPAA compliance, and patient confidentiality standards is necessary. Previous caregiving roles involving patient personal care assistance, mobility support, or hospice care will strengthen your application. Live-in employment experience or working with patients requiring heavy lifting or specialized equipment like Hoyer lifts is a plus.
