Job Overview We are seeking a dedicated and compassionate Caregiver/Home Health Aide to join our team. The ideal candidate will possess a strong commitment to patient care, with experience supporting individuals with diverse needs including seniors, those with disabilities, or individuals requiring memory or dementia care. This role involves providing personalized assistance that promotes independence while ensuring safety and comfort. Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote dignity and independence. Monitor patients' health status by observing vital signs, reporting changes, and maintaining accurate documentation in compliance with HIPAA regulations. Support medication administration and ensure adherence to prescribed care plans under supervision. Facilitate mobility and transfers using appropriate techniques such as Hoyer lifts or patient transfer methods to prevent injury. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Provide companionship and emotional support while engaging clients in meaningful activities to enhance mental well-being. Maintain a clean, safe environment by performing light housekeeping tasks including cleaning and sanitation in accordance with health standards. Assist clients with transportation for medical appointments or community outings, ensuring safety at all times. Requirements Prior experience working as a caregiver or home health aide in settings such as group homes, assisted living facilities, nursing homes, or home care environments. Knowledge of patient observation techniques, vital signs monitoring, medication administration, and first aid/CPR certification. Familiarity with caring for individuals with Alzheimer's disease, dementia, autism spectrum disorder, intellectual disabilities, or neurodevelopmental disorders. Ability to perform manual handling tasks including lifting and the use of assistive devices like Hoyer lifts safely. Strong communication skills for effective client interaction and documentation of care activities. Experience working within HIPAA guidelines to ensure client confidentiality and privacy. Valid driver's license or reliable transportation for client outings is preferred but not mandatory.
