We are seeking compassionate, reliable, and motivated Habilitation Providers to join our growing team. Job Responsibilities Provide one-on-one support to members in their home and community settings Assist individuals in developing daily living skills and independence Support personal goals outlined in individualized service plans Promote social engagement and community integration Assist with communication, problem-solving, and decision-making skills Provide transportation to community activities and appointments as needed Maintain accurate documentation and service notes Ensure member safety, dignity, and well-being at all times Qualifications Must be at least 18 years old Valid driver's license, reliable transportation, and current auto insurance Ability to pass background screening requirements Strong communication and interpersonal skills Experience working with individuals with developmental disabilities is preferred but not required Compassionate, dependable, and professional attitude Why Join Tridev Healthcare?
