Responsibilities:

Provide personal care services, including bathing, dressing, grooming, and toileting. Assist patients with mobility and transfers, ensuring their safety and comfort. Perform light housekeeping tasks to maintain a clean and safe environment for patients. Prepare meals and assist with feeding as needed. Monitor patients' conditions and report any changes to the supervising nurse or healthcare professional. Offer companionship and emotional support to patients and their families. Follow care plans and document services provided accurately and timely. Collaborate with healthcare team members to ensure coordinated and holistic care. Maintain confidentiality of patient information and adhere to all healthcare regulations.

Home Health & Hospice Aide Qualifications:

Current Home Health Aide (HHA)/ CNA certification in the state of CA. 1-2 years of experience in home health or hospice care, preferably in a similar setting. BLS/CPR certification required issued by AHA. Valid CA Driver's license and reliable transportation.
