We have about 5-10 visits per week. You must have a Home Health Aide Certification through the California Department of Public Health. The impact you will make:

Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient's needs and as specified on the CHHA Assignment. Ensure quality and safe delivery of CHHA services per written plan of care. Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient's condition and/or needs.

Who you are:

Completion of an approved training program pursuant to Federal/State law. Current, active California CHHA certificate with criminal background check. Current CPR, health clearance and completed skills assessment. Full job description available upon request
