In this vital role, you will deliver essential personal care and support services to our clients, helping them live comfortably and safely at home. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Provide supervision of a behavioral health client. This is a 2 staff to 1 client environment Monitor patients' health status by observing vital signs and recognizing changes in condition Support medication administration following care plans and HIPAA regulations for patient privacy Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Provide patient transfer assistance using manual handling techniques or equipment such as Hoyer lifts and patient transfer methods Maintain a clean, safe environment by performing light cleaning duties and ensuring infection control standards are met Document patient care activities accurately in compliance with healthcare regulations and communicate effectively with clients and team members Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care settings such as nursing homes or assisted living facilities Strong knowledge of patient observation, monitoring vital signs, medication administration, and dementia or Alzheimer's care Familiarity with care plans, HIPAA compliance, manual handling equipment, and patient transfer methods Excellent communication skills for engaging with clients, families, and healthcare teams Ability to perform heavy lifting safely and assist with assisted living or home health tasks efficiently Experience working with individuals with autism or developmental disabilities is highly valued Certified in First Aid and CPR; additional certifications related to hospice care or home health are a plus Join us at Fresno Yosemite Health Care where your dedication transforms lives!
