Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and personal hygiene Monitor patient health by observing vital signs, medication adherence, and overall condition, reporting any concerns promptly Support patients with mobility using transfer methods such as Hoyer lifts or patient transfer techniques to ensure safety during movement Prepare nutritious meals and assist with feeding as needed, adhering to care plans and dietary restrictions Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Provide companionship and emotional support to reduce feelings of loneliness or isolation Document care activities accurately in compliance with HIPAA regulations and agency policies Skills Experience working with individuals with disabilities, neurodevelopmental disorders, or developmental disabilities is highly valued Knowledge of Alzheimer's care, dementia care, memory care, and hospice care best practices Familiarity with assisted living environments, nursing homes, group homes, or home health settings Ability to observe patients closely for changes in condition or behavior and communicate effectively with team members and families Proficiency in patient monitoring techniques including vital signs measurement and medication administration Skilled in manual handling techniques such as patient transfers using Hoyer lifts or other assistive devices Strong caregiving skills including assisting with personal care tasks like toileting, mobility support, and feeding Knowledge of safety procedures including CPR and first aid certification is preferred Ability to work independently while following detailed care plans and protocols Excellent communication skills to foster trustful relationships with clients and their families Join us in providing compassionate home care that empowers individuals to live safely and comfortably in the environment they cherish most.
