This vital role offers an engaging opportunity to provide personalized assistance, promote independence, and enhance quality of life for residents in a Memory Care settings.
- Assist residents with ADL's (Activities of Daily Living) as identified in the service plan.
- Assist residents with dressing and undressing, as needed, in clothing appropriate to the time of day and season of the year.
- Assisting the client with use of the commode, urinal, or toilet and with adult incontinence products as assigned.
- Assist residents in transferring from bed, chair, wheelchair, toilet, etc., using appropriate equipment and following principles of body mechanics (determine number of persons for assist; use gait belt, Hoyer lift, grab bar, etc.).
- Assist in use of hearing aids and glasses for residents.
- Help each resident to reach and maintain the highest level of performance and independence.
- Provide residents with both physical and emotional support.
- Observe residents and report changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.
- Assist with activities by encouraging activity participation and escorting residents to scheduled activities.
- Participate in activities as assigned.
- Interact with residents to provide reassurance, engagement, and redirection as appropriate.
- Communicate resident information to the Memory Care Director and Wellness Director verbally and through the daily log.
- Complete all required documentation daily; service schedule, flow sheet, communication book, etc.
- Attend In-service training as assigned.
- Assist residents with before and after meal routines (wash hands, face, transport to/from dining area or prepare for in-room dining).
- Provide assistance with serving meals and snacks, feeding residents - if needed, and providing adequate hydration (record intake/output and observations as directed).
- Record bowel habits and urine outputs as directed.
- Laundry as needed soiled linens and clothes.
