This vital role offers an engaging opportunity to provide personalized assistance, promote independence, and enhance quality of life for residents in a Memory Care settings.

• Assist residents with ADL's (Activities of Daily Living) as identified in the service plan.

These cares may include all or some of the following: assisting with dressing, grooming and oral cares; shampooing of hair; assisting with showering or bathing; rubbing with lotions; fingernail cares and foot cares, denture care, skin care, cosmetics, toileting, and bathing if necessary; redirection of confused client, and other cares as assigned.

• Assist residents with dressing and undressing, as needed, in clothing appropriate to the time of day and season of the year.
• Assisting the client with use of the commode, urinal, or toilet and with adult incontinence products as assigned.
• Assist residents in transferring from bed, chair, wheelchair, toilet, etc., using appropriate equipment and following principles of body mechanics (determine number of persons for assist; use gait belt, Hoyer lift, grab bar, etc.).
• Assist in use of hearing aids and glasses for residents.
• Help each resident to reach and maintain the highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Observe residents and report changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.
• Assist with activities by encouraging activity participation and escorting residents to scheduled activities.
• Participate in activities as assigned.
• Interact with residents to provide reassurance, engagement, and redirection as appropriate.
• Communicate resident information to the Memory Care Director and Wellness Director verbally and through the daily log.
• Complete all required documentation daily; service schedule, flow sheet, communication book, etc.
• Attend In-service training as assigned.
• Assist residents with before and after meal routines (wash hands, face, transport to/from dining area or prepare for in-room dining).
• Provide assistance with serving meals and snacks, feeding residents - if needed, and providing adequate hydration (record intake/output and observations as directed).
• Record bowel habits and urine outputs as directed.
• Laundry as needed soiled linens and clothes.
