In this role, you will assist clients with daily activities, monitor their health and well-being, and ensure a safe, comfortable environment. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately Support medication administration following care plans and HIPAA regulations to ensure patient privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Operate patient transfer equipment such as Hoyer lifts and utilize proper manual handling techniques for safe patient movement Maintain cleanliness of client living spaces through regular cleaning and sanitation tasks Provide transportation assistance for appointments, errands, or community activities while ensuring client safety Skills Prior experience working in Seniors, assisted living facilities, nursing homes, or home health environments is highly desirable Strong patient observation skills with the ability to recognize changes in condition or behavior promptly Knowledge of dementia care, Alzheimer's care, memory care, and hospice care practices Familiarity with care plans, medication administration, patient monitoring, and vital signs measurement Ability to perform personal support tasks such as assisting with ADLs and providing compassionate caregiving for individuals with disabilities or neurodevelopmental disorders Proficiency in using patient transfer methods like Hoyer lifts and understanding manual handling safety procedures Excellent communication skills for effective client interaction and collaboration with healthcare teams Join us in delivering exceptional care that truly makes a difference!
