Duties Conduct thorough psychosocial assessments and develop individualized care plans for patients receiving home health services Provide mental health counseling, including clinical counseling, behavioral therapy, and crisis intervention tailored to diverse populations Coordinate community resources to support patients' social determinants of health, such as housing, transportation, and financial assistance Assist with discharge planning by evaluating patient needs and collaborating with healthcare teams to ensure seamless transitions from hospital or inpatient settings Document all patient interactions accurately within electronic medical records (EMR systems), adhering to medical documentation standards and HIPAA privacy regulations Deliver specialized services including hospice & palliative care support, addiction counseling, Alzheimer's and dementia care, PTSD care, and autism spectrum disorder interventions Facilitate individual and group therapy sessions utilizing evidence-based practices like cognitive-behavioral therapy (CBT) and motivational interviewing Engage in ongoing clinical research initiatives and data collection efforts to improve service quality and patient outcomes in home health settings Experience Proven experience working with geriatric patients, individuals with disabilities, or those requiring post-acute or hospice care Demonstrated knowledge of social work practice standards, medical terminology, ICD coding, Medicare regulations, and clinical ethical guidelines Hands-on experience in inpatient settings such as hospitals or nursing homes is highly desirable Familiarity with EMR systems and medical records management is essential for efficient documentation and case management Background in crisis management, behavioral health counseling, mental health therapy, or outpatient clinic experience is preferred Experience working with diverse cultural backgrounds and individuals with developmental disabilities enhances your ability to deliver culturally competent care Prior involvement in research projects related to clinical social work or healthcare outcomes is a plus Join us to make a lasting impact on patients' lives through compassionate care and expert support.
