In this vital role, you will provide personalized assistance to patients in their homes, supporting their health, safety, and well-being. Your expertise will help clients maintain independence and improve quality of life while adhering to all home health regulations and standards. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions, including vital signs and overall health status, reporting any changes promptly Administer medications and perform basic medical procedures such as catheter care and wound management under supervision or according to care plans Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and supportive environment Document medical records accurately using Electronic Health Record (EHR) systems while maintaining compliance with HIPAA regulations Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Utilize equipment like Hoyer lifts or other assistive devices to safely transfer patients and prevent injury Provide emotional support and companionship to promote mental well-being during visits Requirements Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in accordance with state regulations Proven experience working in home healthcare settings or assisted living facilities preferred Knowledge of medical terminology, medication administration, and basic life support (CPR/First Aid) certifications required Experience caring for geriatric patients or individuals with developmental disabilities is highly desirable Ability to perform heavy lifting and physical tasks safely while maintaining patient comfort Familiarity with EMR/EHR systems for accurate documentation of patient care activities Strong communication skills to collaborate effectively with clients, families, and healthcare teams Commitment to respecting patient privacy by adhering to HIPAA guidelines and home & community care standards Join us in providing compassionate, expert care that empowers our clients to live healthier, happier lives at home.
