Job Description:

Provide direct patient care in a home setting, ensuring the comfort and well-being of patients Assit patients with personal care (bathing, grooming, dressing) Help with mobility and transfers Evaluate home environment for hazards or barriers to more independent living. Monitor vital signs and report changes to nurse/ supervisor Educate and instruct patients, family members, or other patient representatives, in safety Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Requirements:

Valid Home Health Aide (HHA) in the state of California Minimum 1 year of experience as an HHA in Home Health Ability to lift/move patients safely Proficient in taking and interpreting vital signs Strong communication and interpersonal skills Part time position
