Job Description

Direct Support Professional- Direct Support Professionals will provide services to the individuals in the home to meet the individual's needs. These staff will assist the individuals in living a life that is as independent and rewarding as possible while maintaining the individual's rights. The services provided shall include and not be limited to; high quality care, community inclusion, assistance with cooking, cleaning, and activities of daily living, medication assistance and nurse delegation services. These individuals will be supervised by the House Administrator or Designee. Essential Duties and Responsibilities Is a mandated reporter of suspected abuse and will follow all guidelines established to meet this requirement Provides supervision and training of individuals in the home Maintain attendance records and all required documentation and reports per agency procedures. Maintains records and documentation of progress, follow-up and completes data collection on a regular basis as required by Supervisor, Licensing, and Regional Center Participate in the individual's assessment planning and evaluation process. Provide assistance, support, and motivation as necessary for individuals in the home Assist individuals with their self-help needs, including support in the areas of grooming, hygiene, toileting, feeding and mobility Maintain consumer and program confidentiality Implement and maintain individuals program plans Maintain a professional relationship with employers, individuals, parents and care providers and others as appropriate Report any actual or potential conflicts to a supervisor Attend and participate in all required meetings and training sessions Demonstrate on a daily bases the organization's Vision, Mission and Core Values Ensure safety and welfare of staff and individuals and follow adopted procedures in order to report accidents, hazards, incidents, or unsafe conditions to immediate supervisor in a timely manner Performs other duties as assigned.
MINIMUM QUALIFICATIONS
High School Diploma or Equivalent At least 18 years of age Valid Driver's License Vehicle insurance Obtain California Criminal Clearance Live Scan Lines of Supervision Under supervision of the House Administrator/designee will be responsible for providing direct supervision and assistance to the individuals while following their IPP based on their current Needs and Service plans. Will report any and all issues to the Administrator and appropriate outside agencies.
