$17.00/ HR Job Summary The Certified Home Health / Hospice Aide (CHHA) is a paraprofessional member of the home health and hospice care team who works under the supervision of a Registered Nurse or Therapist. The CHHA provides personal care services to patients in their homes to meet individual needs, promote comfort, and support quality of life. Key Responsibilities Assist patients with personal care needs to promote comfort and safety Observe patient condition and promptly report changes to the supervising clinician Accurately document care provided and patient observations Required Skills & Experience Ability to work effectively with a diverse patient population Strong interpersonal and communication skills (oral and written) Patient and family sensitivity with a non-judgmental, empathetic approach

Education Required:

High School Diploma or equivalent Required Certifications & Licensure Certified Home Health Aide (CHHA)
