As a vital member of our healthcare team, you will deliver personalized assistance that promotes independence, safety, and well-being. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following home healthcare regulations and medical documentation protocols, ensuring proper medication management and adherence to physician orders. Provide specialized care for clients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques and behavioral management strategies. Maintain patient confidentiality in compliance with HIPAA regulations while communicating effectively with patients, families, and healthcare professionals. Experience Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred. Demonstrated experience in geriatric care, long-term care settings, or nursing homes is advantageous. Proven ability to work with individuals with developmental disabilities or disabilities requiring specialized attention. Familiarity with medical terminology, catheter care, wound care, and basic life support (BLS) certifications such as CPR are required. Experience using EMR/EHR systems for documentation and client communication is preferred. Knowledge of home health care regulations and best practices for patient transfer methods is essential.
