We treat each of our patients as individuals, creating the most comfortable and dignified end-of-life care that's right for them. We are seeking a Hospice Home Health Aid for routine care visits. Responsibilities include following the plan of care, providing comfort, and working collaboratively with the members of the team to help meet positive patient care outcomes. Essential Job Functions/Responsibilities Perform personal care needs after given a patient's assignment. Report to the Case Manager at the end of each visit, prior to departing from residence. Provide patient and family with confidentiality and maintain patient's dignity. Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) Give personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as assigned in CHHA assignment. Assist in dressing and undressing patients, as assigned. Assist in feeding the patient, as assigned. Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Report all changes to Case Manager, prior to departing from residence. Assess and report on patient's condition and significant changes to the Case Manager, prior to departing from residence.

Qualifications:

Hospice Experience Valid and current CHHA certification Possess and maintains current CPR certification. Reliable transportation

Pay:

$40.00 per hour
